a disclosure is a document containing information held by police and government departments. it can be used by employers and voluntary organisations to make safer recruitment and registration decisions. the disclosure service offers organisations a means to check the background of applicants to ensure that they do not have a history that would make them unsuitable for posts they are trying to fill. disclosures will provide details of a person's criminal record including convictions, cautions, reprimands and warnings held on the police national computer (pnc). if the position involves working with children, disclosures will also contain details from lists held by the department of health (dh) and the department for children, schools and families of those considered unsuitable for this type of work. how do i apply for a disclosure? to apply for a disclosure you should select 'registration page' on the home page of this site and complete the form to register for your cqc check. please note at this stage in the process we only require first & last names and not titles, apostrophes, single letters or abbreviated names. cqc will then send an email with your username and password to log in to the full application form. they aim to do this within 48 hours, however due to high volumes of requests it may take longer. once you have received your username and password you can log in to the form by clicking on 'login page' on the home page. the username and password fields are case sensitive and so it is recommended that you copy and paste the information with no spaces either side to avoid errors. you will then need to enter your security answer and give your consent before continuing with the form. what information will i be required to include on my disclosure form? in completing the form you are required to include: your full name, and any other name that you may have been known details of addresses where you have lived during the past five years in addition you will be required to provide personal identification documents including items such as your passport and/or driving licence. these id documents must all be in the same name and at least 1 item needs to confirm your present address. it would also be advised to have these documents ready before completing the form as it does not save until you reach the end. the information requested is required by the dbs to ensure that they are able to confirm and verify your true identity and current address. you should note that in providing this service the dbs and cqc are committed to compliance with the data protection act. this means that any personal information submitted will be protected. for more information about data protection please see the link in the menu area. what if my job title is not stated on the application request? to get clarity on the job roles we suggest contacting the cqc on 03000 61 61 61 before completing the request. what happens when i have completed my online disclosure application form? when you have completed the form you will be prompted to print a referral letter. this must be taken to one of the post offices that offers the service (please see the link at the bottom of this page) in the allocated time stated in the letter. you must also take the fee stated in the letter and all identification used within the application for verification. please note that only the applicant applying for the dbs can have the referral letter verified at the post office. who will receive my disclosure? you will receive your disclosure certificate in the post from the dbs. it will be sent to the current address as stated on your application. cqc will not receive a copy of your result but they may be required to see yours - if this is necessary, you will be contacted by post. what if i misplace my disclosure? if you misplace your dbs certificate and have not already registered with cqc, you will need to request a replacement from dbs. if this is not available you will need to re-apply for login details as your dbs certificate may be required at your fit person’s interview. how long does the process take? as the process relies on an initial cqc verification and a full verification at the dbs including police checks there is no specific time frame as each applicant’s information is different. can i challenge the information on my disclosure if it is incorrect? yes. if you believe the personal information on your disclosure is incorrect, please contact the dbs on 03000 200 190 and select option 2 or alternatively, you can complete section a of the 'certificate dispute form'. if you wish to challenge information relating to criminal records or other information disclosed, the dispute must be made in writing and section b of the 'certificate dispute form' must be completed. the form can be found on the dbs' website via www.gov.uk/dbs. you should also advise cqc if you are disputing the information on your certificate so they are aware of additional delays in seeing your certificate. if i have a criminal record will i be able to register with cqc?
